Hello, I am doing a nursing course and I have to participate in a discussion board, by replying to one of my colleague’s POST (or her mini essay) by saying my opinion, that I agree.

With reference to the above clip discuss what constitutes a safe discharge for Mrs A with Congestive Heart Failure, she was seen today by the medical team who say ‘send her home today, she’s ready for discharge her daughter can hold the fort!’. Would you send her home?
What do you think is the nurse’s role in the discharge process? Reflect on and discuss Mrs A’s requirements in order to facilitate a safe discharge?
I have already read my colleague essay and I totally agree with what she is saying. You have to start the essay by saying: ‘’I agree with you that….’’ (whatever she mention in her essay). And then support what she is saying.

Below is my colleague POST (or her mini essay). please agree with her and say way.
When a patient is admitted on the ward in the hospital setting, the nursing team assess the patient, schematise the care plan, and highlight all necessary interventions to meet the patient healthcare needs. And together with a multidisciplinary team, decide a provisional discharge date, taking into consideration patient’s presenting and potential health problems.
According to Dougherty and Lister (2008), discharge planning was defined as a process and not a single isolated event. The Department of Health (2010) mentioned that the discharge process involves the development and implementation of a plan to make sure there is a suitable setting available in the community, whether it is the patient’s own home or a residential care home, for the patient to be discharged from hospital. This process must be based on initial assessment undertaken during patient’s admission (DH 2010)
Regarding Mrs A. case, she was admitted following an episode of dizziness which caused her to fall and fracture her hip in her own home where she lives alone. The fracture was healing following a successful surgery. Mrs A. has a medical history of heart failure disease, which might have caused dizziness, leading to her falling and fracturing her hip. Following her vital signs observation records, there was no issue with her blood pressure, which prompted the medical team to decide her discharge, despite important medical tests results still unavailable to influence the decision.
The medical team decision to discharge Mrs A. did not consider all the steps of a good quality discharge process, which ensures the patient is safe to be transferred from hospital to the community; ensures the patient and carers including family are involved in the discharge process; provides patient and carers with written and verbal information to meets their needs and provides the continuity of care between the hospital and her home (DH 2004, cited in Dougherty and Lister 2008).
The role of the nurse in this process must be based on effective communication
in nurse- patient relationship, and ensure all members of an MDT and family members (Mrs A’s daughter) involved in the discharge process agree on the decision to let Mrs A. home.
All admitted patients, whether have acute or long term condition, should receive a full and complete discharge planning (Dougherty and Lister 2008).
For Mrs A. case, the nurse should consider her particular care needs regarding that she lives alone, she is elderly person, she has a limited prognosis, and she might require equipment aid at home before she is allowed to leave the hospital (NHS, 2008)
So, as the Department of Health stated, the discharge planning takes into account the patient’s physical, psychological, social, cultural and economical needs. And this was overlooked in Mrs A. case. Although the cardiologist has increased one of Mrs A’s anti-arrhythmia medications , the medial team should have waited for the MDT’s opinion and results of her chest X-rays requested by the pulmonologist, which might have influenced the decision otherwise. Thus, the nurse looking after Mrs A. must intervene and request for her to be reviewed by the MDT and medical team to make sure she is fit to be discharged.
