2) Select images linked to the brand (e.g. adverts / logo’s / packaging / web pages)
3) Analyse the images, paying specific attention to positioning, meaning, gender, culture, colours
4) Additional marks will be given for use of credible (academic) background research to support your findings
5) Students striving to achieve higher marks are also advised to apply academic theories/frameworks where relevant

Marking Criteria

Presentation (10 marks)
Work must be presented in a professional, academic way. This will include visual presentation such as headings and page numbering as well as a formal written tone.

Supporting content (10 marks)
This typically acknowledges aspects such as an informative introduction, reflective conclusion and addition of key definitions where relevant.

Selection of effective materials (10 marks)
Students will be rewarded for effective choices with regards to brand imagery. Materials should enable sufficient discussion on the topic areas discussed in class. Fewer marks will be awarded for images which provide a restrictive scope for discussion.

Analysis of materials (25 marks)
This involves dissecting the visual content of the collected materials and applying it to concepts covered during classes. The function and form of each image should also be addressed. Applications of each concept to the image should be fully explained and justified.

Use of secondary literature (25 marks)
Higher marks will be awarded for the inclusion of academic sources to underpin the analysis of materials. A more advanced analysis will include comparing, contrasting and critiquing of academic studies and/or theoretical frameworks.

Originality (10 marks)
Originality will be demonstrated by showing evidence of independent thinking. Students can show originality by going above and beyond the basic expectations for the assignment.

Strategic Analysis Report through Tesla case study .

Strategic Analysis Report through Tesla case study .
TESLA STRUCTURE

Executive Summary (excluded from the word count)
Yes, it must be before the table of contents! It can be max one page and it is excluded from the word count, so please be analytic about it. Use it to impress us, make it professional. Here you need to include all the major points from every part of your report. Again, this is not an introduction, it is an executive summary

Table of contents
1. Introduction
Since you have limited word count, the intro should be very small – max one paragraph. Here you simply introduce the reader in what is this report about and what you will analyse. – It is different from the ex.summary.

2. Q1 – Analysis of the external environment
Sub-headings here e.g. 1.Macro Environment Analysis – PESTEL, 2 Micro Environment Analysis – Porter’s 5 Forces Model
Q1 – 20%:
Here you are analysing the external environment of TESLA market/industry– thus the focus is not TESLA – the focus is the micro (5 Forces) and macro (PESTEL-) environment. That means both PESTEL and 5 forces should not be about TESLA but for the industry of TESLA… Plenty of you were asking whether you should focus on UK or other countries or globally. My answer is that it is up to you. Preferably I would like to see either an external environment analysis for UK, mentioning also some essential global trends OR you can make a European/Global analysis straightforward. In any case, you will need to make that explicitly clear in your executive summary and introduction, state in which level you make your external environment analysis.

Also, here you will need first to analyse PESTEL – just mention only those relevant trends to your industry . You don’t have to go into describing every dimension. If you want to and already done it – then use tables and/or appendices and mention in the text only the most important trends.

5Forces: Use the “Structural Determinants” slide from the external environment lecture slides – 1st semester. Please mention whether every force is high, moderate or low and in the end conclude whether the industry is attractive and profitable or not.
As to industry life cycle (ILC): You can either clearly incorporate it within the “existing competition” force of 5 forces, by saying whether the industry is in a development or maturity stage along with the market growth rate (which a structural determinant of the particular force) OR you can make a short section only for the industry life cycle theory.

In any case, you will need to reference every single concept and model used above both in text and in the final list of references.

3. Q2. – Analysis of the internal environment
Sub-headings here: e.g. 1.Resource Based View of the Company, 2 Value Chain etc..
Q2-20%:
In this question we expect you to use resources and capabilities theory (R+Cs or else Resource Based view of the Company) and/or Value Chain . Yes, please, mention both theories and reference them of course. However, if you feel comfortable using only one then it is still fine – just state it clearly. So, we would like you to: present the key strategic resources. Please see examples of resources and capabilities from page 94 of our book and lecture slides. Within the financial resources you must mention some key financial indexes either of TESLAs’ Strategic Business Unit (SBU) (and/)or TESLA’s group that you can find from Google, published statements, Mintel or Bloomberg databases. As for mentioning comparative measures from a key competitor, yes you can do it. More than that, you will need to state clearly what are the core capabilities/competencies of TESLA and why they are core? E.g. financial control and economies of scale-are they core? Also do they lead to competitive advantage? If yes, is it cost leadership or differentiation or a bit of both (hybrid strategy)? In this way you are actually describing TESLA’s existing business level strategy that you need in the last question.

As for Value Chain. You don’t need to analyse highly descriptive every single activity. If you want to or have done so, please use appendices and tables to avoid the word count. Here, you need to show those linkages (combinations of primary to support activities) that either increase the customer value or reduce the operating cost. In this way, again you can conclude into what competitive advantage(s) are created for TESLA and how. The outcomes of Resources+Capabilities and Value Chain should agree.

The above are the essential ones that we want for Q2.

As for leadership, decision making, culture, politics etc. Yes, you can mention them within the resources appraisal above! OR, if you want to make a separate section/part for those (since you have a limited word count), please keep it short and practically focused. In any case, please reference properly/academically every single of those concepts. The essential part of this question is R+Cs and Value Chain.

NOTE ABOUT SWOT: We didn’t and we don’t want you to do a SWOT analysis. Essentially you have analysed OT from Q1 and SW from Q2. If you want or have already done a SWOT please use is as framework to summarize your key points from questions one and two. You can put them in a table after Q2 and before Q3.

4. Q3. – Analysis of PR crises
Subheadings again here – according to your identified crises/relevant theory
Q3-10%:
In this question you are required to identify one, two or three issues that affected the image of the company, either internal or external. You will need to briefly analyse what happened applying theories on organisational ethics theory,, corporate governance CSR concepts, stakeholder applications & reputation management concepts/metrics. Not all of them are required, but the more you apply and reference inside the PR crisis/issue the better it is. We want you to demonstrate application of relevant theories.

5. Q4. – Strategic Analysis
Subheadings here: e.g. 1. Business Level Strategy, 2. Corporate Strategy, 3. Strategies Suggested for future/Evaluation, 4. Implementation Modes/Techniques and Final Suggestions.
Q4 – 30%
This is obviously the most important question. Here you need to:
a) Identify the business level strategy of TESLA (this could be also been found/linked from question 2. That is either cost leadership or differentiation or hybrid (both). Of course you will need to reference all concepts such as “Porter, 1985”.
b) Identify the corporate level strategies. These could be growth: e.g. market penetration, market development, product development, vertical/horizontal integration, related/unrelated diversification. Here you can use the Ansoff’s matrix/directional policy table (it is essentially the same). Of course there are turnaround corporate strategies (e.g. downsizing, liquidation, divesture etc) and stability strategies ( no-change strategy, pause, etc)
So in a) and b) you can identify/record plenty of strategies but please use the terminology and reference properly every concept.
c) Suggest strategies: Either you can suggest sustaining the existing ones, or developing the existing ones, or developing new.
d) Choose two (advisable)-three (max) strategies to evaluate them according to certain criteria. These can be:
– suitability – acceptability – feasibility, that is SAF (highly recommended, simple and straightforward)
-STAIR model
-SUITS model
-RACES model
– or any other properly referenced model.

You don’t need to use more than one of the above

6. Conclusions – Yes, this can be as part/sub-heading of the previous point – it is up to you.
7. Full list of References / Bibliography (excluded from the word count)
8. Appendices. (excluded from the word count)

And here are some extra guidelines;

Examining the historical lens of gifted education

The purpose of the thesis:
is therefore to explore the balance of influences (Western and Islamic) that Saudi needs for a sound education policy and implementation strategy in the area of gifted education. As such, it will engage in a literature review of gifted education in Western scholarship, uncovering major themes, and then attempt to find similar themes in traditional Islamic scholarship. In addition, it will explore the contribution of Islamic scholarship generally in Western scholarship in order to illustrate that there is a history of interface between the two scholarships.

In order to achieve the objectives, the thesis hopes to find a measure of parity and a mutual relationship between the two scholarships, such that Saudi education can both apply Western research about gifted education and, at the same time, re-discover some of the roots of its own Islamic scholarship around the issue of giftedness. The study will employ qualitative methodology, specifically document and conceptual analysis, and forms of historiography in order to achieve its goals.

So, this task need to look at the comments, with considering and doing the following:

The major problem with this chapter is the” writing way”, it is not an academic writing, “PhD thesis” the main comments about it are the following:
1. Introductions in each section, needs to give the reader an idea where are we? and what we doing here?
2. Linking between the paragraphs and the sections.
3. The ideas are there but not clear. Need to highlight and write about them again and again.
4. Delete these not related ideas or statements, or paragraph.
5. Weak of referencing, add solid references to each Concept,”Theme” to support the importance of these concepts “themes”.
The concepts”themes” are 5 major concepts ”themes” as the following:
1. Knowledge, Needs 3 new solid references. 2. Intelligence, Needs 3 new solid references. 3. Wisdom. Needs 3 new solid references. 4. Emotional intelligence. Needs 5 new solid references 5. Spiritual intelligence. Needs 5 new solid references 6. Moral intelligence. Needs 5 new solid references
6. Arguing weak and not supporting with evidences!
7. Change the words from “theme” to concept.

In this stage” this chapter” have to focus and answer the following research questions:

1. What are the major educational concepts, between Western scholarship and traditional Islamic scholarship?
2. What is the contribution of Islamic scholarship in Western scholarship?
3. What contribution to thinking about gifted education comes from a traditional Islamic perspective.
4. What is the balance of influences (Western and Islamic) that Saudi educational system needs for a good policy on gifted education?

Assignment Task
This assignment requires you to identify an organization reported on in the literature or
in the public domain and report on its use of training and development. The case study
should highlight how training and development has impacted or is impacting on the 9
organisation’s alignment of corporate strategy and human resources strategy. The
theoretical model underpinning this case study is the positive correlation that has been
reported in the literature between training and development (T&D) on the one hand,
and the ability to implement corporate strategies on the other. The link between
corporate strategy and T&D is the “Human Resources Strategy”. Your research needs to
identify whether such as link exists and, if so, how T&D can positively impact the
implementation of the corporate strategy. To do this, you will need to undertake
research into the topic and use references to justify your response for the company you
have selected.
Examples of Learning and Development Plan and of Employee Training Plans will be
discussed during the first and second lectures in class (weeks 1 and 2).

Information Systems Analysis & Modelling

It requires you to develop an information system
model for a system of your own choice. The aim is to get you to apply many of the system analysis and
modelling techniques discussed and illustrated in lectures and tutorials to a real, or at least realistic, case of
non-trivial but still reasonable and do-able complexity.
Choosing your system
As stated in the Introduction, the choice of which system to analyse and model is up to you. However, here
are some rough guidelines to assist you in your choice:
1 Try to choose a system for an area or application about which you know a considerable amount or
about which you are in a position to find out what you need to know. Examples might be a system
related to your current work (if you are presently employed) or previous work experience, a hobby or
interest that you have, or perhaps a system that you or a colleague or friend needs. My experience
in running earlier versions of this course shows unambiguously that this assignment will work best
(and you will learn the most from it) if you choose to analyse and develop a model for a system that
relates to the “real” world in some way, rather than one that is completely fictional and invented
entirely “within your own head”.
2 Try to choose a system of a reasonable size and level of complexity. This will probably be hard to
judge initially, but it is generally better to choose a system that is more likely to turn out to be too big
or complex rather than one that may turn out to be too small or simple. The reason for this is that, if
your choice does turn out to be too big or complex, you can always reduce the scope of your
intended system or choose to model only part of it. On the other hand, if your choice turns out to be
too simple and small to form a useful assignment exercise, it is generally harder to expand it to make
it more suitable as a worthwhile learning experience as well as the basis for an acceptable
assignment submission. Although we won’t discuss these metrics until mid-way through the course
(so they will be less useful to you at the beginning), one useful indicator of size and complexity of a
project is the number of entities in its data model. If this turns out to be somewhere around 6-8 for
your project then you probably have a suitably sized system for this assignment, although these
numbers should definitely not be treated as hard limits. However, if the number of entities in your
data model is significantly fewer than 6 then the model is probably too small and simple to be a
useful learning exercise; and if it is many more than 8 then it is probably starting to get too large to
feasibly tackle. Another indicator is the number of levels you find yourself going down to in your DFD
hierarchy. If this is more than two for the first few processes you decompose, and there are more
than about seven processes on your level 0 diagram, then your chosen system is highly likely to be
too big and you may have to reduce its scope or simply leave parts of the model incomplete.

Your tasks and deliverables

Your second assignment deliverable is a mid-project progress report. This is to be provided digitally via
Wattle and carries a weighting of 10% of the total project mark. In fact, calling this deliverable a “progress
report” is somewhat misleading since it is not to be a formal report describing what progress you have made
to date but is rather to show what progress you have made by providing all of the (no doubt unfinished,
incomplete, draft and still developing) analysis work that you have done on the project so far. And again, like
the first deliverable, what you provide here will form the basis on which you will build the relevant sections of
your final assignment deliverable (see below).
The third and final deliverable for this assignment, worth the remaining 85% of the total project mark, is a
report (submitted digitally via Wattle) containing the following:
1. An Introduction that describes the overall background and rationale for the system you have chosen
to model. In writing this Introduction you should assume that your reader has little or no knowledge
of the application area and system with which you are dealing and this Introduction should therefore
take your reader to a point where she/he has enough information to fully understand all the material
that follows. This will, of course, be based on what you provided for the first project deliverable (see
above).
2. A section describing the scope, functions, constraints, and any other relevant features that apply to
the system you have chosen to model. Be careful, in writing this as well as the following section, to
avoid focussing on matters that are primarily technical. Remember that it is analysis you are
supposed to be doing, not design or implementation, and this is largely a business-oriented activity
dealing with what the system should be doing rather than how it is eventually going to do it.
3. A section presenting the detailed user requirements that form the basis for your model of the system.
While it is not always possible, generally the best projects are those that have real potential users
(other than you!) for the system you are analysing and modelling and with whom you need to interact
to develop the list of detailed system user requirements.
4. A section documenting the actual models you have constructed. This is to include process, logic and
data models. The process model is to be constructed using the DFD technique and should, at least
for several processes, extend all the way down to a primitive DFD together with its associated logic
model. The data model is to be constructed using the ER diagramming technique. You are also to
provide a data dictionary in which the meanings of all the important terms in your model are
explained.
5. A section in which you provide either a UML version of your system model or a “Soft” analysis of the
context in which your system development would take place, whichever is more appropriate for the
system with which you are dealing in your project.
NOTE: If you believe it is appropriate, you may include both a “Soft” analysis and a UML version of
your system model. If you do opt to include both, you will be eligible for bonus marks up to 10% of
the value of the whole assignment. That is, if you provide both a UML version of your model and a
“Soft” analysis for your project then your maximum possible mark for the project as a whole will be
66 rather than 60. A word of warning is appropriate here, however: I will be more impressed by a
project that does a good and thorough job of the required work as opposed to one that attempts
everything, and as a result does a poorer quality job on all of it. A good quality basic assignment

without the inclusion of the optional extra material will most likely earn more marks than a poorer
quality one that attempts everything.
6. Lastly, a section in which you reflect on your analysis work and discuss what you learned about the
analysis process as well as any difficulties you faced and how you overcame them, and any other
interesting points that emerged from your assignment work. Please also include, in this section, your
thoughts on the value of the assignment as a learning tool for this course, and any way(s) in which
you think it could be improved.

Other Remarks
If you have difficulty in deciding what would be an appropriate system to model, or are having other trouble
deciding what to do, please discuss it with me before you set out too far in your work.
You may use an appropriate CASE tool to assist you in your work, in fact, it is STRONGLY recommended if
you can obtain or have access to one, but it is not essential for the purposes of this assignment. As the work
needs to be submitted digitally via Wattle uploaded, any hand-drawn materials will need to be scanned and
included in your assignment upload file (Word document or PDF).

Instruction: Students must submit a clinical problem or PICOT question with 4-6 abstracts to the course instructor for an approval prior to proceeding with the EBP project. Students must follow specific requirements in this guideline to receive a full credit.

I. Introduction (25) (Estimated length: 1-2 paragraphs)
•Describe a specific clinical practice background that leads to PICOT question (5)
•Articulate the significance of the clinical problem and the need for an EBP investigation by 1 or more of the following methods (5):

o Use of evidence to establish the prevalence/incidence of a specific clinical problem/situation

o Use of evidence to establish the risks associated with a specific clinical problem/situation

o Use of evidence to establish the potential impact of a specific clinical problem/situation on clients’ quality of life
•Introduce the clinical question and set the stage for the subsequent EBP inquiry (5)
•Engage the reader and link to the clinical practice (5)
•A clinical question must be written clearly in a PICOT format with all five components identified, patient/population, intervention, comparison (if applicable), outcome, and time (5)

II. EBP Investigation Process (25)
◦Presentation of evidence investigation is objective, written in past tense, uses appropriate tentative language and active voice, third person, and utilizes the following subheadings:

§ Selection Criteria (5): (Estimated length: 1 paragraph)
◾State the specific criteria you established and used for selection of “best evidence”
◾Describe the selection criteria used and ensure they are appropriate for PICOT question

§ Search Process (15): (Estimated length: 1 paragraph)

§ Describe the search process in details that you followed in your comprehensive review of the literature relevant to selected topic. Provide rationales for the final selection of the studies included for the best evidence.

§ Include details about databases, source of evidence searched, the specific MeSH terms/key words utilized in your search process, and the specific types of evidence targeted in your search process.

§ Describe details of search results.

§ Selection of Best Evidence (5): (Estimated length: 1 paragraph)

§ Identify the specific studies/publications, which you selected as “best evidence”. Refer your readers to the matrix in the Appendix A for a summary of each selected evidence document.

§ Assure your readers that you selected all or a sufficient proportion of applicable best evidence, which met your pre-established selection criteria without bias.

§ Discuss/explain the reasons behind any exclusions of located applicable evidence (as appropriate).

III. Critical Appraisal of Collective Evidence (20): (Estimated length: 3-5 paragraphs)

§ Use appropriate critique templates according to the type of the study design provided at the bottom of this guideline to evaluate each selected study. Critique each selected article and organize templates in Appendix B.

§ Offer your critique of selected collective (not individual) evidence through the use of synthesis. Examples of aspects, which are typically critiqued, include the adequacy of sample sizes, appropriateness of methods for research question, bias demonstrated in inclusion/exclusion criteria, data collection, interventions, recency of research, and the level of significance of study results. For example: “All four of the studies implemented and intervention of vaccination with the HPV vaccine in young women and compared this to the placebo group…. Except for Smith et al’s study group (2009) which only included vaccination protection from type 16 and 18, the other studies included protection against type 6, 11, 16, and 18. Although the Smith et al’s study group (2009) investigation excluded protection in the vaccine from types 6 and 11, results showed cross protection from types 31, 33, and 45 which are also oncogenic.”

§ Evaluate the applicability of selected collective (not individual) evidence to clinical question/situation through the use of synthesis. Examples of aspects, which are typically evaluated, include the congruency/match between the selected evidence and clinical question in terms of PICO components—population, intervention, comparison, and outcome and the quality of the study. The purpose of this evaluation of applicability is to show the reader that application of the findings from the selected best evidence to your EBP clinical problem/situation is warranted/valid.

· Evaluate the sufficiency of selected collective (not individual) evidence to address the clinical problem/situation through the use of synthesis. Included in your evaluation of the sufficiency of collective evidence is your perception of the need (or lack of need) for further research.

IV. Collective Results (20): (Estimated length: 2-3 paragraphs)

§ Provide a summary or synthesis of the collective results of the EBP investigation (What is new or currently known about this topic?)

§ Offer a critical analysis of the research findings by comparing and contrasting the quality of research across collective studies including statistical findings and p-values or confidence interval.

§ Draw conclusions from the collective results of the examined evidence through the use of synthesis. The following example illustrates a summary of results and conclusions drawn from the synthesis of collective results:

Two systematic review studies and a case control study, which compared large samples of cases with ovarian cancer with large control groups did not find a greater risk of ovarian cancer development in individuals who breastfed their infants for 6 to 12months (Klaus, Penington, & Sim, 2006; Leawood et al., 2004; Mena, et al., 2008). In contrast, Bar-Yam, Smith, and Young (2009) in a pooled analysis of 2 case control studies with a smaller sample size found a greater risk for the development of ovarian cancer among non-breastfeeding women. A greater risk for the development of ovarian cancer was noted among those who did not breastfeed (Bar-Yam, Smith, and Young (2009).

V. Intervention Decision (10): (Estimated length: 1-2 paragraphs)

§ Discuss the implications for practice that can be drawn from the results of your EBP inquiry. What specific “evidence-based” practice intervention(s) are supported (not supported) by your findings? Are there specific parameters that should be followed in the implementation of supported intervention(s)?

§ If evidence did not support the intervention or insufficient evidence, research to generate new evidence may be needed. The implications should be discussed with respect to future research and recommendation for the next steps.

§ Explore and select an EBP model to guide the implementation of the project. Briefly describe steps of the model.

§ Explore how the values/preferences of “others” (patient, administrators, team members) may influence the implementation of the supported intervention (if applicable)

§ Explore your ability to implement the “evidence-based” intervention in terms of the available resources within clinical settings.

VI. Plan for Implementation and Evaluation of Intervention Decision or a brief research plan (if evidence of intervention was not supported) (10) (Estimated length: 1-2 paragraphs)
•Based on the above intervention decision and your chosen EBP model, outline a specific plan for the implementation of an “Evidence-based” intervention in clinical practice or brief research plan that may include: design, setting and sample, data analysis, and time frame.
•Clearly state description and protocol of your intervention. Be sure the effectiveness of the interventions are appropriately measured, e.g., Client’s cholesterol level will remain within X and Y.
•Delineate your plan for measurement of outcomes (e.g., frequency/duration of measurement). For example: Client’s cholesterol level will be monitored monthly for the first six months following initiation of intervention.
•Identify any data that you will used in order to evaluate the effectiveness of your intervention decision.

Directions: Complete matrix for each of the studies/publications you have selected as “best evidence” for your PICOT question.

Below is an example of the Summary of Review of Literature Matrix

PICOT question: Among adult intensive care unit (ICU) patients who require mechanical ventilation (P), does the use of oral care using chlorhexidine (I) compared to the usual care, placebos, or no intervention (C) reduce the incidence of the development of ventilator-associated pneumonia (VAP) (O)?

Munro et al., (2009).

Chlorhexidine, toothbrushing, and preventing ventilator-associated pneumonia in critically ill adults.

Method: 2 x 2 factorial design, level 3

Examine the outcome of toothbrushing alone, chlorhexidine alone, or a combination on the incidence of VAP

Mechanically ventilated adults admitted to three ICUs

IV= chlorhexidine

DV= Incidence of pneumonia

Pneumonia developed in 24% by day 3 in patients treated with chlorhexidine with a CPIS ≥6. When data on all patients were analyzed together, mixed models analysis indicated no effect of either chlorhexidine (P=.29) or toothbrushing (P=.95). However, chlorhexidine significantly reduced the incidence of pneumonia on day 3 (CPIS ≥6) who had a CPIS < 6 at baseline (P=.006). Toothbrushing has no effect on CPIS and did not enhance the effect of chlorhexidine. A randomized trial of chlorhexidine gluconate on oral bacterial pathogens in mechanically ventilated patients (Scannapieco et al., 2009) Method: Randomized controlled trial. Double-blind, placebo-controlled, level 2 To identify the minimal effective frequency of 0.12% CHX to reduce colonization. Mechanically ventilated pts admitted to the trauma ICU IV= chlorhexidine DV= Incidence of pneumonia No statistical significant results were noted, all p≥0.05 in regards to reduction of plaque scores between all groups (p=0.8423), reduction of potential respiratory bacterial pathogens (PRPs) p=0.6750, delay of pneumonia (hazard risk= 0.555, 95% CI, 0.256-1.201, p=0.1348), or incidence of pneumonia (RR=41%, OR=0.54, 95% CI, 0-23-1.25, p=0.1459). Although, not statistically significant, trends of reduction in of S. aureus, delay of pneumonia, and incidence of pneumonia (7 in qd IG, 7 in bid IG, 12 in CG) were noted between the intervention groups and the control group. Randomized controlled trial and meta-analysis of oral decontamination with 2% chlorhexidine solution for the prevention of ventilator-associate pneumonia (Tantipong et al., 2008) Method: randomized controlled trial with stratified randomization, level 2 To evaluate the impact of 2% chlorhexidine use to delay or inhibit VAP. Mechanically ventilated adult patients in ICU or medical wards IV= chlorhexidine DV= Incidence of pneumonia Incidence of VAP in CHX-4.9% and 11.4% in saline group (p=0.08) Appendix B: Critique Template Directions: Complete critique template for each of the research articles that you have selected as “best evidence” for your PICOT question. To identify the minimal effective frequency of 0.12% chlorhexidine gluconate (CHX) to reduce colonization. What research methodology was used (e.g., RCT, Case-Control, Cohort)? Randomized controlled trial. Double-blind, placebo-controlled Describe the sample (including size)? 175 total sample (58 in intervention group qd, 58 in intervention group bid, and 59 in control placebo group). How was the sample selected (e.g., randomization, inclusion/exclusion criteria)? Randomized sample with a block design. Inclusion criteria: ICU admissions, and intubation with mechanical ventilation w/in 48hrs of admissions. Exclusion criteria: witnessed aspiration, diagnosis of post-obstructive pneumonia, hypersensitivity to CHX, no consent, diagnosis or thrombocytopenia, do-not-intubate orders, younger than 18yrs.of age, diagnosis of pregnancy, legal incarceration, transfer from another ICU, oral mucositis, immunosuppression, or re-admission to the ICU. What methods were used for data analysis? Computerized quality assurance checks, ANOVA, chi-squared tests, Kaplan-Meier method, and Cox proportional hazards model. What were the results of the study? At what Level of Significance? No statistical significant results were noted, all p≥0.05 in regards to reduction of plaque scores between all groups (p=0.8423), reduction of potential respiratory bacterial pathogens (PRPs) p=0.6750, delay of pneumonia (hazard risk= 0.555, 95% CI, 0.256-1.201, p=0.1348), or incidence of pneumonia (RR=41%, OR=0.54, 95% CI, 0-23-1.25, p=0.1459). Although, not statistically significant, trends of reduction in of S. aureaus, delay of pneumonia, and incidence of pneumonia (7 in qd IG, 7 in bid IG, 12 in CG) were noted between the intervention groups and the control group. What conclusions/implications for practice were drawn by the authors? The intent of this study was to compare the frequencies of CHX. The sample size was estimated to be adequate for the three comparative groups of frequency and placebo. There were no real differences noted between once daily vs. twice daily application of CHX. Although the sample size wasn’t statistically adequate in terms of comparing CHX alone vs. placebo, it does infer that CHX is an effective intervention to prevent VAP. It is also effective in decreasing colonization of S. aureaus; however, not effective in decreasing colonization in biofilm d/t the properties of biofilm and plaque and it’s resistance to antimicrobial agents. No adverse effects were noted from CHX treatment, and no differences seen in length of days of ventilations, length of hospital stay, or mortality. Your Critical Appraisal of the Study’s Appropriateness for Application to Clinical Practice Was the research design appropriate for the research question (e.g., RCT for cause-effect questions, Case Control for harm or causation questions, Cohort for questions regarding prognosis)? Support your response. The research design (randomized controlled, double blind-placebo) was appropriate for answering intervention/effectiveness questions. The only better design would be systematic reviews or meta-analyses which would be a higher evidence level. Was the sample size adequate? Was bias controlled with sample selection methods? Support your responses. The sample size was adequate (minimal 53 participants per group) for the question of the most effective frequency; however, it wasn’t adequate to provide statistically significant findings in terms of the EBP question of CHX on VAP. Bias was limited with randomization, block design, and blinding. Were the study results significant (level of significance)? Are the results valid and reliable? Is there a possibility of bias or of confounding variables? Support your responses. The study results were not statistically significant; however, clinically significant for the EBP question in regards to VAP reduction. Results are valid and reliable with CI of 95%. There are some confounding variables that can introduce bias such as the disproportionate representation of African Americans, Asians, and American Indians among the groups. Different ethnicities may have different characteristics in terms of oral colonization amounts. Also, some co-morbidities were disproportionate among groups (i.e. renal/hepatic) that could play a part in immunosuppression. Is the study and its findings directly applicable to your EBP question (match all selection criteria)? Support your response. The study matches the EBP in terms of population (adult ICU patients who are mechanically ventilated >48hrs), and intervention (chlorhexidine use in oral care). However, the outcomes differ. The findings are clinically significance in terms of incidence of VAP and some reduction of colonization.

Your Summary of the Study/Publication (use paraphrasing, not quotes)

What was the stated research question/objective of the study?

To determine the best evidence available for providing oral care to mechanically ventilated ICU patients, and to provide a evidence-based plan to improve patient outcomes.

What search strategies were used to locate applicable studies for possible inclusion?

CINAHL, Medline, Joanna Briggs Institute, Cochrane Library, Embase, DARE, and Google.

What were the stated inclusion/exclusion criteria?

Both experimental and non-experimental study designs were included due to the limited review material on ICU patients. The review included 11 prospective controlled trials, 20 observational studies and 24 descriptive reports. The studies included ICU patients who were mechanically ventilated, and studies that proposed a link between oral hygiene and systematic diseases. The interventions of interest were those designed to affect dental plaque specifically and oral hygiene in general. Exclusion criteria: studies that included healthy participants, and studies that were done in a setting other than a critical care environment.

What was the total sample size (e.g., how many studies were included, number of subjects)?

Fifty-Five studies were included with a resultant sample of 3,415 participants in the 11 RCT’s. The review did not discuss how many participants were in the other studies.

What were the results of the study?

Overall, there was a reduction in VAP, but there is a lack of evidence based research for oral care outcome measures on ventilated patients to determine the effectiveness of oral hygiene interventions.

What conclusions/implications for practice were drawn by the authors?

The study found that there is a correlation between the colonization of dental plaque with respiratory pathogens and the occurrence of pneumonia, and a relationship between poor oral hygiene and the incidence of VAP, but there is limited protocols based on research studies for best practice in providing oral care in the ICU. Oral care may contribute to improved clinical outcomes, but further research is needed to establish best practice guidelines. Chlorhexidine mouth rinse in cardiothoracic ICU patients was shown to reduce the incidence of VAP .

Your Critical Appraisal of the Study’s Appropriateness for Application to Clinical Practice

Is the sample size adequate? Support your response.

Although, the study did not list the total participants from all of the studies reviewed, the 11 RCT studies included 3, 415 participants which is an adequate sample size. However, two of the RCT studies used a small sample.

Are the research methods used appropriate for the study’s research question/objective? Support your response.

The research methods used are appropriate for the study’s research question. The researchers researched peer-reviewed literature to determine the best available evidence for providing oral care to ventilated ICU patients. According to Melnyk and Fineout-Overholt (2011), systematic reviews answer specific clinical questions using methods that reduce bias and draw conclusions about evidence gathered.

Was the completed search systematic, comprehensive, and without bias? Support your response.

The search was a comprehensive and systematic search, and the researchers did not show bias in the articles selected. The researchers used multiple databases to search for evidence and used articles published between 1985 to 2006, and the search strategy was verified by a health librarian.

Were selection criteria clearly described? Are they appropriate? Was bias demonstrated in the study’s inclusion/exclusion criteria? Support your responses.

Selection criteria were used and appropriate for this study. Researchers must specify criteria and consider a resulting sample that represents the population of interest (Polit & Beck, 2008).

Is the research recent enough to be applied to current clinical practice? Support your response.

Yes, the research is recent enough to apply to current clinical practice. The Systematic Review used at least 8 RCT studies from the last 10 years. The study did not specify in detail the years of the other studies, other than mentioning they used published articles from 1985 – 2006.

Were the study results significant? Support your response.

Yes, although study concluded that further research is needed for evidence based practice guidelines on oral hygiene, there was a significant reduction in of VAP in the ICU patients.

Is the study and its findings directly applicable to your EBP question (match all selection criteria)? Support your response.

Yes, the findings are directly applicable. The population included ICU mechanically ventilated patients, used various interventions (chlorhexidine use in oral care, no oral care, toothbrushing alone), outcome (incidence of VAP) are similar to our EBP question. The findings are significant, methodology and sample sizes are adequate, and the highest level of evidence is used.
