Final Project
Please use the grading rubric to create an outline of your assignment. Each section of the rubric should be a section of your final paper and could become the headings. Your assignment will be graded based on each element of the rubric. Compare each section of your paper with the rubric to ensure all elements are covered. Then, include an introduction and conclusion to tie the paper together. If you have any questions regarding the assignment please contact your instructor using the Course Help forum.
For this Part Four of your Project, write a paper in APA format and address:
How you would implement this case management protocol in your own organization or in the community. What approach would you use to get the buy-in of key stakeholders? Key outcomes of this case management plan.
Combine all sections of your course project (including the final section that is due this week) into a single paper before submitting.

Professor announcements
Hello Dear Class, Here are some assignment specific instructions for this course. Any questions please ask! Note a common error is to hand in assignment 4 without the compilation of parts 1, 2 and 3. KDE
Assignment 1
Follow the rubric. Academic, evidence based writing which you are doing now as BSN scholars is not creative writing. You may have great ideas for catchy headings and so on but save them for when you graduate and write an article for publication.
Avoid the temptation to write everything there is to know about your topic. I know as nurses, we become emotional about our work and after seeing patients suffer from COPD, DM and so on, it is frustrating that the world does not take advice seriously about their health care. But this is not the place to change the world. These course assignments are about these rubric points. Again, when you learn good academic, evidence based writing techniques as BSN scholars you can write away on your own and I truly hope you do. We need more politically active nurses.
For now, ASU wants you to learn the rubric points so your headings should reflect those rubric points. And speaking of headings, yes, do use headings. Think of the reader, not as your instructor, but as administration. You really want administration to choose your case management proposal. Perhaps administration wants to read your take on nursing in case management or population, for example, but if they have to peruse paragraph after paragraph searching for that information, your paper loses its effectiveness and is not presenting strong, concise information. And have you ever seen an article without headings?! Think of yourselves as BSN scholars writing an article for publication when doing your assignments. That is not out of the realm of possibilities, you could publish someday!
Your papers should be three to four pages long excluding the title page and reference page. Use the OWL site https://owl.english.purdue.edu/owl/resource/560/03/, your APA 6th edition and/or the APA guidelines found in the announcements to have correctly formatted citations, quotations with page number, and reference list up to par.
Please peruse the announcement with most common APA errors I find so you can avoid them. Note the information on topics in your running head and title and how to put the page number with quotations, for example.
This information is true for not only assignment 1, but for all subsequent assignments. Do read the other assignment instructions with more particulars on those assignments.
Common error with assignment 1 is to not use headings, not stay on task with the rubrics and to have long paragraphs about the topic that do not relate to the rubric points.
Questions always please ask.
Assignment 2
For this assignment on best practices, published guidelines, and clinical pathways you will write a brief synopsis about what each one is as they relate to healthcare. Then you will also mention how they relate to your topic. If your topic is on DM, for example, you will write some info on best practices for diabetes, some examples of published guidelines and clinical pathways.
Common error with assignment 2 is to write a dissertation on the rubric points with no mention of the specific topic.
Questions as always, please ask.
Assignment 3
In the announcements, for your convenience, you will find posted the document of the LifeCareWriterPlan so you do not have to go to the site and obtain theirs. One student, once upon a time, did not read any instructions and paid money to use the document which was never the intention of ASU. Reading the instructions is a time-saver!
Use a patient that you know or that you create. You must put a fictional disclaimer at first mention of any identifying date, name.
You will put your information into this document, taking out the document text, following the headings.
There are many appendices mentioned in the rubric, those are examples, you are not expected to do all of them, but do choose a couple that pertain to your topic.
This assignment is an exercise. It is not going to be an APA masterpiece, follow the format, and correctly use some citations appropriately.
Note that this document has an appendix. Follow the idiosyncrasies of such… If you have an appendix, that appendix will follow the references…I do not find that intuitive but that is what APA says! http://writingcenter.waldenu.edu/1078.htm; http://owl.english.purdue.edu/owl/resource/560/18/; http://uws.au.libanswers.com/a.php?qid=162066
If you have an appendix, be sure you mention that appendix in the body of the paper to alert the reader of its presence. http://www.nova.edu/chcs/studentaffairs/success/forms/apa-appendices.pdf; https://owl.english.purdue.edu/owl/resource/670/05/
Common error with assignment 3 is to not include any financial information, tables per the format nor references.
Questions please ask.
Assignment 4
Note the Module 8 overview…putting it all together. Though the rubric only has the three rubric points for assignment 4, the intention of ASU is that you will now put parts 1, 2, 3 and 4 together to have a final comprehensive paper on case management and your topic.
Correct any errors in parts 1, 2, and 3.
You will edit the introductions and write a new final paper introduction that includes all parts.
You will edit the conclusions and write a final paper conclusion that includes all parts.
You will edit the reference list to one final paper reference list that includes all parts.
The rest of the assignments can remain intact, or you can edit the assignments themselves. However, you would add connecting sentences as needed between the assignment parts you are compiling together.
Cut and paste the assignments 1 through 4 into a final paper document with the newly edited introduction, conclusion and reference page, or rewrite with your edited versions.
It makes the paper flow and read easily if you put assignment 3 as an appendix. Remember the idiosyncrasies of appendices. You would mention in the text of your paper that there is a LifeCareWriterPlan example in the appendix. Then you would have that appendix after the references as per APA links mentioned in assignment 3 information.
Voila’! You are done with Case Management!
Common error with assignment 4 is to only submit part 4 by itself, and not correcting the previous assignments before including them with the final paper creation.
