Purpose

The purpose of this assignment is to provide the student with an opportunity to develop a health promotion program and relate strategies of community planning, implementation and evaluation to the community diagnosis identified in Assignment 2. Please note you are not expected to carry out the health promotion program that you develop.

Choose one of the following to complete this assignment:

Based on the community diagnosis you developed in Assignment 2, (a) develop an outline for a program to meet the identified need, including goals and objectives, and (b) compare and contrast the strengths and limitations of two approaches for influencing community change in the direction of health which would be appropriate in developing the proposed program (e.g., behavioral / lifestyle, socio-environmental approach) and which need to be considered in the planning, implementation and evaluation of the program. Discuss the role of the community health nurse related to each of the approaches and in relation to the planned program.

Or

Based on the community diagnosis you developed in Assignment 2, (a) develop an outline for a program to meet the identified need and (b) develop a proposal for obtaining funding for the program. The proposal should include goals and objectives for the proposed program; a description of the target aggregate/group; rationale for the approach to be utilized (e.g., behavioral / lifestyle, socio-environmental approach); a description of the “players” who will be involved in program planning, implementation and evaluation and their roles: proposed evaluation strategies; and, a budget for the program.

Marking Criteria

All aspects of the assignment as outlined are clearly addressed.

Critical analysis and synthesis of ideas is evident.

Ideas are develop logically and presented in an organized, clear, and concise manner.

Paper will build on course content and will reflect use of resources beyond those included in the course study guide.

APA (6th. ed.) scholarly format – limit of 12 pages (excluding title and reference pages).

The following are some tips for the assignment 5:

– Specify clearly either on the title page or in the introduction what option you will be presenting. If choosing option A, the objective of the paper is to create a program that is based on the results of your assessment in Assignment 2. Then based on the concepts and theory of this course choose 2 approaches and compare and contrast their strength and limitations. There are a few approaches to a program: behavioral / lifestyle, , biomedical, socio-environmental approach or as on page 290 in Community as Partner text (3rd ed.) -Community Social Action, Community Development, Social Planning, Community Organization and Service Extension, and support with literature. Discuss in terms of how the processes and outcomes would have been described, measured, etc. using the different approaches. Include how each approach would influence the choices made within the program. Include what the community nurse’s role would be for each of these approaches as it relates to your program, for example, the nurses role (what would they do differently / how would they approach things differently / what would they evaluate that might be different from the behavioural perspective if a socio-environmental view was taken. Include a definition that is supported by literature.

Give a brief description of the aggregate and specify the one priority community diagnosis that will be used to help develop the program.

(Develop a program outline using the elements of program planning. The program logic model is a useful strategy. See the chapters 15, 16, and 17 in the Canadian community as partner text (2nd ed and 3rd edition) or starting on page 236 in the Stamler and Yui text (3rd ed.) relating to program planning and evaluation for a guide. Including a diagram that represents the program plan in an appendix or figure would make the components of the program clear to potential funders for the program. Include the human, financial, organizational, and community resources a program has currently available that could be directed towards the program implementation. Using a model such as the program logic model to help you create a structure of each of the components in your proposal would add credibility with outside sources; the main elements of the program are captured and it depicts the cause-and-effect relationship between the program and its outcomes. Resources and constraints of the program must be identified throughout the program outline.

?When discussing goals or strategies for the diagnosis, planning the interventions or evaluation for your program, remember the SMART acronym (Specific, Measurable, Acceptable, and Realistic to achieve, and have a Time line) that provides a succinct format and clearly outlines the parameters for the planning implementation and evaluation process. Include with each objective a time frame for attaining the change, the direction and magnitude of the change i.e. immunization levels at school entry will increase to 97%, the method for measuring the change e.g., After the session, each participant will demonstrate…It produces more precise objectives than improved, increase or decrease. These will be important (especially when requesting funding) for outside evaluators as well as the people participating in the program to check on their progress. Also, think of ways to keep track of the processes (the how to piece) of making a program work.

(b) compare and contrast the strengths and limitations of two approaches for influencing community change in the direction of health which would be appropriate in developing the proposed program (e.g., behavioral / lifestyle, socio-environmental approach) and which need to be considered in the planning, implementation and evaluation of the program (See page Chapter 6 in Community Health Nursing 3rd edition by Stamler and Yui or specially pages 90-94. )

Evaluation considerations and indicators are also definitely an important part in program planning for this assignment. You will likely need to address in some ways the process indicators (the how does the program work – and how will you make sure it is working this way) as well as outcome indicators (what do you expect to see if the program works).

-Include the role of the CHN in the process and also how the aggregate would be involved in the planning. Including the aggregate in the validation of the community diagnosis as well as when designing the program is important to include and support with literature.

For Option B tips-

Present the program outline that includes goals and objectives. (Consider using a recognized model; for example, such as the program logic model to help you create a structure of each of the components in your proposal. This adds credibility with outside sources; the main elements of the program are captured and it depicts the cause-and-effect relationship between the program and its outcomes. You can use many other references to present the model or a model you may have used or prefer.

Participation is critical to getting the best results – a lack of participation can lead to decisions being over-ruled, delayed, challenged or questioned by internal or external stakeholders.

Resources and constraints of the program must be identified throughout the program outline.

Include: a description of the “players” who will be involved in program planning, implementation and evaluation and their roles: proposed evaluation strategies; and, a budget for the program

Discuss the rationale for the approach to be utilized (e.g., behavioral / lifestyle, socio-environmental approach)(or as on page 290 in Community as Partner text (3rd ed.) -Community Social Action, Community Development, Social Planning, Community Organization and Service Extension) and support with literature.

The program planning process, implementation and evaluation and their roles: proposed evaluation strategies; and, a budget for the program ” the whole paper could be presented as a proposal that includes the outline with the goals as well as meeting all the points on the marking criteria. The proposal should also include a table with the proposed financial statement of the costs and in-kind contribution that could be specified in an appendix.

Ensure all the points are addressed on the marking criteria and that you use a variety of references to support the discussion. I look forward to reading the final paper.

Whatever option chosen before initiating a program, consider if the program focuses on community health. This means seeking to improve the health of groups of people taking into context their family and their community to benefit the quality of life for the community as a whole (Wollman Anderson & McFarlane, 2012).
