Based on the community diagnosis you developed (a) develop an outline for a program to meet the identified need, including goals and objectives, and (b) compare and contrast the strengths and limitations of two approaches for influencing community change in the direction of health which would be appropriate in developing the proposed program (e.g., behavioral / lifestyle, socio-environmental approach) and which need to be considered in the planning, implementation and evaluation of the program. Discuss the role of the community health nurse related to each of the approaches and in relation to the planned program.
Assignment 5 just flows from the Assessment Assignment 2 – so having the legwork done in the assessment makes the rest easier. The objective of the paper is to create a program that is based on the results of your assessment in Assignment 2.
Clearly identify the aggregate from assignment 2 in a short concise statement as well as include the focused community diagnosis that is related to the identified need for this aggregate (that is specified the 4 parts as indicated by the Canadian Community as Partner Text. Page 281).
Develop a program outline using the elements of program planning. Using a recognized framework or model such as the program logic model to present the outline of the program, adds credibility with outside sources for acceptance and funding.
Evaluation indicators are definitely a piece of assignment 4. You will likely need to address in some ways the process indicators (the how does the program work – and how will you make sure it is working this way) as well as outcome indicators (what do you expect to see if the program works).
For objectives (outcome short/ long term or process objectives specify using the SMART formula (-specific, measurable, action oriented, realistic, and time specific). Indicators need to be specified for each objective), activities (actions mapped out in sequence to deliver program to reach goals), resources (list to successfully carry out the intervention eg. personnel, funding, materials, training, promotional expenses) Integrate program planning with evaluation.
Option A tips-
One way to approach this paper is to start with the introduction that states the purpose of the paper and strategy. Present a concise aggregate description and diagnosis from assignment 2. Then present the program outline.
Based on the concepts and theory of this course choose 2 approaches for Option A and compare and contrast their strength and limitations.
Then compare and contrast the strengths and limitations of two approaches for influencing community change in the direction of health which would be appropriate in developing the proposed program (e.g., behavioral / lifestyle, socio-environmental approach) and which need to be considered in the planning, implementation and evaluation of the program. Discuss in terms of how the processes and outcomes would have been described, measured, etc. using the different approaches. Include how each approach would influence the choices made within the program.
Include what the community nurse’s role would be for each of these approaches as it relates to the planned program, for example, the nurses role (what would they do differently / how would they approach things differently / what would they evaluate that might be different from the behavioural perspective) if a socio-environmental view was taken?.
For Option B
One way to approach this paper is to start with the introduction that states the purpose of the paper and strategy. Present a concise aggregate description and diagnosis from assignment 2. Then present the program outline; then include the proposal. The paper is in APA format and the proposal can be written within the body of the paper. Essentially this whole paper is a proposal that includes all the components in the marking criteria. Timelines and an evaluation throughout the program planning implementation and outcomes are important to include.
The rationale for the approach chosen can be included in your proposal. Include how the selected approach influences the choices made within the program. Include what the community nurse’s role would be for this approach as it relates to your program, for example, the nurse’s role and approach to the program, what would be evaluated using this approach. Discuss in terms of how the processes and outcomes would have be described, measured, etc. using the specific approach.
Include all the proposal criteria as specified.
In conclusion both Option A and B need to address all the key components in the marking criteria as it relates to planning, implementation and evaluation of a program that meets the aggregate’s needs identified in assignment 2.
The following are the marking criteria for each Assignment 5 option:
Marking Criteria -Option A
1.All aspects of the assignment as outlined are clearly addressed. Based on the community diagnosis you developed in Assignment 2
(a) develop an outline for a program to meet the identified need, including goals and objectives (/5)
(b) compare and contrast the strengths and limitations of two approaches for influencing community change in the direction of health
? which would be appropriate in developing the proposed program (e.g., behavioral / lifestyle, socio-environmental approach)
? and which need to be considered in the planning, implementation and evaluation of the program. (/5)
? Discuss the role of the community health nurse related to each of the approaches and in relation to the planned program. (/5)
2.Critical analysis and synthesis of ideas is evident. (/5)
3.Ideas are develop logically and presented in an organized, clear, and concise manner.
4.Paper will build on course content and will reflect use of resources beyond those included in the course study guide. (/5)
5.APA (6th ed.) scholarly format – limit of 12 pages (excluding title and reference pages
Marking Criteria -Option B
1. All aspects of the assignment as outlined are clearly addressed: Based on the community diagnosis you developed in Assignment 2
(a) develop an outline for a program to meet the identified need and (/5)
(b) develop a proposal for obtaining funding for the program. (/10)
The proposal should include:
? goals and objectives for the proposed program;
? a description of the target aggregate/group;
? rationale for the approach to be utilized (e.g., behavioral / lifestyle, socio-environmental approach);
? a description of the “players” who will be involved in program planning,
? implementation and evaluation and their roles:
? proposed evaluation strategies;
? a budget for the program.
2. Critical analysis and synthesis of ideas is evident. Ideas are develop logically and presented in an organized, clear, and concise manner. (/5)
3. Paper will build on course content and will reflect use of resources beyond those included in the course study guide. (/5)
5.APA (6th ed.) scholarly format – limit of 12 pages (excluding title and reference)
